DVDxDV is not free, but it's very good and very fast. It shows a list of all the titles, and from there you can select an in-point and an out-point. When you extract the movie, it lets you pick quicktime settings from the standard codec window. This makes for nice, one-step ripping. Also rips from VOBs in case you are ripping an encrypted DVD.
